  • Overview

    General Purpose of the Position:

    The Client Care Coordinator is responsible for the coordination of admissions and case management services within a substance abuse facility that provides Residential, Partial Hospitalization, Intensive Outpatient and Outpatient Services. The Client Care Coordinator completes assessments, insurance and FMLA documents and provides discharge and aftercare services. The Client Care Coordinator communicates with law enforcement and probation, employers and government agencies on behalf of the client and builds relationships with community partners to provide clients with referral and linkage to community resources. The Client Care Coordinator acts as a liaison between the multidisciplinary team and other departments, families and other outside agencies to advocate for the client and remove barriers to care.

     

     

     

     

    Responsibilities

     

     

    Responsibilities:

    • Coordinates admissions and case management services
    • Provides orientation services to acclimate client to milieu
    • Performs behavioral interviews toward the completion of the Admissions and Case Management Assessments
    • Provides discharge and after care services with guidance from the clinical team
    • Assist staff in coordinating all admissions and discharges
    • Creation of Client Charts in the EMR
    • Provides assistance with filing for disability, food stamps, FMLA and other services
    • Communicates with attorney’s, probation and parole officers, employers and other entities
    • Assists with ACA (Against Clinical Advice) blocking and client management
    • Liaises with the on-site multi-disciplinary team, as well as the Utilization Review Team, Business Development and other departments
    • Leads groups in support of programming needs
    • Participates in case staffing’s and reports concerns
    • Attends daily staff meetings and weekly department meetings as pertinent to job function and responsibilities.

    • Works with administration to enhance the treatment environment
    • Accepts guidance form the Program Manager, Clinical Director and Leadership Team
    • Demonstrates effective writing skills, including proper grammar and spelling
    • Participates in professional development activities, such as field training, certification and education
    • Possesses treatment knowledge related to Substance Use Disorders and Mental Health conditions

    Computer Equipment and Software Requirements:

    • Word, Excel, Google Docs/Chrome and KIPU EMR

    Educational Background and experience:

    • High School or Bachelor’s Degree with relevant certification (CAC, CAP and/or other)
