  • Overview

    General Purpose of the Position:

    The Group Facilitator is responsible for running groups and performing a variety of patient care activities as directed by the Clinical and Program Directors.  They will be responsible for the care of assigned patients and will provide a variety of topics in a group therapy setting. Facilitators are responsible for documents having to do with the group and the patients who were present.  Additionally, Group Facilitators may assist in conducting bio-psychosocial assessments, developing Master Treatment Plans and assist in coordinating discharge planning.

     

    Responsibilities

    Essential Duties and Responsibilities:

    • Working with Directors, the Group Facilitator helps to create group content.
    • Group Facilitator facilitates each session and keeps careful documentation. 
    • Creates group session curriculum in alignment with Program and is flexible when speaking to topics that are of current need of the facility
    • Group Facilitators are responsible for completing the following
      • curriculum creation for group topics
      • record and note-keeping 
      • Potentially the psychosocial assessment & psychosocial update, and educational learning assessment.
      • Understanding master treatment plan, progress notes and treatment plan review 
      • discharge or transfer summary and a discharge note
    • Attends daily staffing meetings per schedule.
    • Shares information with team as obtained from patient’s family, or others as indicated throughout program.
    • Documents in the medical record on an ongoing basis
    • Works with administration to enhance the overhaul treatment environment
    • Other duties as assigned by supervisor and/or other management.

      • Possess the ability to provide effective treatment at multiple levels of care (ASAM). 

     

    • Demonstrate effective writing skills; proper grammar and spelling. 

     

     

    Qualifications

    Knowledge, Skills and Abilities Required:

    • Excellent computer skills, including Word, Excel, PowerPoint, and Publisher in a Microsoft Windows environment.
    • Effective oral and written communication skills.
    • Strong knowledge of employment laws and practices.
    • Excellent interpersonal skills.
    • Skills in database management and record keeping.
    • Able to exhibit a high level of confidentiality.
    • Excellent organizational skills.
    • Must be able to identify and resolve problems in a timely manner. Critical thinking skills are necessary.

     

    Computer Equipment and Software Requirements:

    • Microsoft Word, Excel, Outlook, PowerPoint.
    • Kipu or similar software

     

    Education and Experience Required:

    • Bachelors Degree in a health related field, CAC, or CAP.
    • Minimum of 1 year in a counseling setting
    • Knowledge of DCF Guidelines
