Overview

The Client Care Coordinator is primarily responsible for coordinating admission, discharge and all level of care transitions throughout the treatment process.  The Client Care Coordinator will also act as a liaison between the multidisciplinary team and other departments, families or outside agencies to advocate for the client to obtain needed treatment or to remove barriers to continuing care.

Responsibilities

  • Conducting assessments performing behavioral interviews
  • Works directly with patients, families and professionals to facilitate the client’s admission, discharge, and other level of care transitions
  • Responds to client concerns
  • Participates in the case staffing and reports concerns from the multi-disciplinary team to other departments as needed
  • Liaisons with utilization review, admissions, business development and other departments and agencies
  • Identifies and meets with clients at high risk for relapse or leaving treatment unsuccessfully
  • Relates to patients easily, shows empathy and builds trust and rapport
  • Builds cohesion among the community and motivates patients for treatment and recovery
  • Prepares educational and motivational lectures, groups & distributes literature as needed
  • Assists clients with aftercare planning and links clients to resources within the community
  • Arranges transportation and coordinates facility transfers
  • Maintains the confidentiality of all clients and facility activities
  • Complies with the organization’s Policies and Procedures
  • Accepts and utilizes supervision
  • Addresses problems noted by supervisor
  • Performs other assignments as requested by supervisor
  • Demonstrates willingness to accept responsibility
  • Completes all tasks with concern for work quality

Qualifications

  • Communication and Written Skills
  • Ability to multi-task
  • Work as a team member
  • Knowledge of Patient Confidentiality Laws
  • Knowledge of Drug-Free Workplace Policies
  • Knowledge of Workplace Violence.
