Overview

Clinicians are responsible for performing a variety of patient care activities as directed by the Program Director.  They will be responsible for the care of assigned patients and will provide group, individual, and family therapies. Clinicians will conduct bio-psychosocial assessments, develop Master Treatment Plans, and will assist in coordinating discharge planning.   

 

Responsibilities

    • Begins, completes, and documents on the initial treatment plan, master treatment plan, integrated diagnostic summary and treatment plan reviews/updates.
    • Completes screenings and assessments of new patients
    • Clinician is responsible for completing the following
      • orientation note
      • initial treatment plan
      • psychosocial assessment & psychosocial update, and educational learning assessment
      • master treatment plan, progress notes and treatment plan review
      • Discharge or transfer summary and a discharge note
    • Develops individualized treatment plan with each patient
    • Performs individual therapy with patient/family
    • Attends weekly staffing meetings per schedule
    • Shares information with team as obtained from patient’s family, or others as indicated throughout program
    • Documents in the medical record on an ongoing basis
    • Provides education to patient/family on community resources
    • Assists patient/family with appropriate discharge plans
    • Works with administration to enhance the overhaul treatment environment 
    • Other duties as assigned by supervisor and/or CEO, COO, CFO or other management.
    • Weekend Coverage.

 

 

Qualifications

 

  • Education: Master’s degree in a health related field required. 
  • Experience: Minimum of 1 year as a practicing counselor in an addiction/recovery/detox setting.
  • License and/or Certifications: See “Education” above
  • Skills:

 

    • Knowledge of DCF guidelines
    • Demonstrates Proficiency in Communication & Written skills
    • Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws
